Our hours of operation are:
Monday: 9:00 am to 6:00 pm
Tuesday: 9:00 am to 5:00 pm
Wednesday: 9:00 am to 5:00 pm
Thursday: 9:00 am to 5:00 pm
Friday: 10:00 am to 4:00 pm

Driving Directions:

From Portland:
Head West on 26W through the tunnel until you get to 217. Take the 217 exit and go South on 217. The second exit is Canyon road. Turn Left at the bottom of the exit ramp onto Canyon road (you are now headed East on Canyon). You will pass under the freeway you just exited. The first stop light you come to beyond the exit ramp will be 110th Ave. Turn Right (South) onto 110th. The road will continue for approximately one block before it splits in a 3-way intersection. At this point you will be looking directly at the front of the building. You will need to branch to the right in order to pull into the parking lot.

From I5 South:
Take I5 North to 217. Take exit 292A which is 217 toward Tigard/Beaverton. Take exit 2A (Beaverton Hillsdale Highway and Canyon road). Turn Right on to Beaverton Hillsdale Highway and proceed to first stop light which is 110th Ave. Turn Left onto 110th Ave. Drive past Home Depot and Chuck E. Cheese. We will be located on the right hand side just as the road curves to the right at 4085 SW 109th Ave. (Note: 109th and 110th run parallel and then join in a 3-way intersection about a block south of Canyon Road. We are located at that junction, so even though our address is on 109th, you can access our building from 110th.)

From West (Hillsboro or the Beach):
Head East on 26W to 217. Take the 217 exit and go South on 217. The second exit is Canyon road. Turn Left at the bottom of the exit ramp onto Canyon road (you are now headed East on Canyon). You will pass under the freeway you just exited. The first stop light you come to beyond the exit ramp will be 110th Ave. Turn Right (South) onto 110th. From North (Vancouver or North Portland):
Head South on I5 to 405S. Take 405S to 26W. Head West on 26W through the tunnel until you get to 217. Take the 217 exit and go South on 217. The second exit is Canyon road. Turn Left at the bottom of the exit ramp onto Canyon road (you are now headed East on Canyon). You will pass under the freeway you just exited. The first stop light you come to beyond the exit ramp will be 110th Ave. Turn Right (South) onto 110th. The road will continue for approximately one block before it splits in a 3-way intersection. At this point you will be looking directly at the front of the building. You will need to branch to the right in order to pull into the parking lot.

The new patient Intake Form will provide us with your contact information as well as a brief health history so that we can conduct the initial visit efficiently and address your entire symptom picture. The last page of the intake form makes reference to our Privacy Practices. You can view those here if you like, or a hard copy in our office. We do not need a printed or signed copy of the entire practices document, just the final page of the intake form that indicates that you have read them.

If you believe that your insurance will cover your visit with one of our doctors, please fill out our Insurance Benefits form. This form will guide you through the investigation with your insurance company to see if your plan includes naturopathic benefits. If you do have coverage, we would be more than happy to bill your insurance company for the visits.

If you believe that you have valuable medical records, such as recent lab work or imaging that would assist us in providing you with the best possible care, please fill out our Request of Records form and mail or fax it to your other health care providers office.
